Caregiver – Floor Lead

Provides support to the case management team regarding the coordination and delivery of services to maximize member participation in inclusive activities both in the facility and the community. Directly supervises Program Assistants staff including hiring, performance improvement, annual reviews, and staff schedules. Ensures in-house activity schedules are completed, compliant with Home and Community Based Services benchmarks, and offered to all participants. Performs other duties as required.


Assists program assistants in providing supportive and safe environment to individuals with disabilities.

Assists in making behavioral observations and implementing individualized person-centered plans and reports their progress.

Prepares and coordinates members person-centered plan goals, and if deemed appropriate, behavioral approaches consistent with individual’s defined outcome.

Maintains accurate daily records of performance; confirming that documentation meets program expectations.

Supervises assigned program assistants who provide direct service including hiring, onboarding and orientation, performance improvement, annual reviews, and staff scheduling.

Monitors and documents member’s progress and critical events relative to their involvement.

Consults with VPI’s RN and case managers to promote wellness and meet personal care needs. In addition, employee provides assistance upon need in meeting members’ personal care needs including feeding and toileting.

Professionally communicates with guardians, parents, supervisors, residential staff and/or other VPI staff to facilitate mutual program planning.

Performs break, lunch, back door arrival duty and other duties assigned.


Work performed includes sitting, standing, walking, crouching, kneeling, stooping, lifting/carrying, pushing/pulling, reaching at/below shoulder level, and reaching overhead. Functions are considered light to moderate according to the US Department of Labor.  


High School diploma or equivalent, an associate degree preferred.

Minimum of two (2) years’ experience working with developmentally disabled, special needs individual.

Demonstrated leadership skills.

Salary: $18.00 – $20.00 per hour
