Case Manager – Waupaca (Treehouse) – TEMP POSITION

Provides employment skills training and support to program participants, as well as overall development and monitoring of individual goals and person-centered plans. Documents activities in a web-based case management system. Effectively collaborates and coordinates services with long-term care, other service providers, schools, customers and employers, and community partners. Communicates relevant issues and progress to VPI Leadership.


1. Schedules and conducts six-month staffing meetings and participant reviews and prepares all necessary documentation.

2. Utilizes assessment information and previous goals to implement a person-centered plan based on participant’s needs and abilities, and documents progress toward goals.

3. Provides goal-oriented, individualized interventions to participants which includes, but is not limited to, behavior management and de-escalation techniques and training.

4. Works closely with production staff in coordinating work opportunities, job set-up, jigs, and job modifications and maintains awareness of production activities. Coordinates and conducts training opportunities as necessary.

5. Professionally communicates with guardians, parents, supervisors, resource workers, group home staff, employers, schools, and other VPI staff to facilitate mutual program planning.

6. Trains participants on proper and safe work techniques.

7. Conducts evaluations prior to entry of new participant at Sturm Foods location.

8. As needed, performs production supervisor responsibilities.

9. Performs other duties as required.


Functions are considered light to moderate according to the US Bureau of Labor Statistics. Work performed includes the ability to move freely (standing, stooping, walking, bending, reaching, pushing, and pulling), and lift to a maximum of fifty (50) pounds without assistance.


Normally works in a climate-controlled environment. Frequent sitting and standing with movement throughout spaces. Use of computers throughout workday. Frequent use of keyboard with repetitive motion of hands, wrist, and fingers.


1. High school diploma required, associate’s or bachelor’s degree preferred.

2. Experience working with individuals with intellectual and developmental disabilities and/or mental health diagnoses preferred

3. Ability to obtain and maintain CPR and First Aid certification – will provide training

4. Available to work on-site during work hours (Monday through Thursday, 9am to 5pm; Friday 7:30am-3:30pm)

5. Computer skills with experience in Microsoft Office preferred.

6. Valid driver’s license and ability to travel between sites (Treehouse Foods, formerly Sturm Foods in Manawa and Creative Converting in Clintonville) as needed – mileage reimbursement provided

PAY: $18.00-$20.00
