JOB SUMMARY:
The Outreach Specialist plays a key role in supporting adults throughout their involvement with the Outreach Center (ORC). Their primary responsibility is to foster meaningful relationships by providing effective coping strategies, modeling healthy interpersonal dynamics, and promoting improved socialization within peer-supported networks.
ESSENTIAL FUNCTIONS:
- Plans and facilitates groups related to mental health, social skills, relationships, independent living skills, vocational activities, etc.
- Assists members with planning outreach events, recreational outings, volunteer activities, and social events.
- Makes referrals for individuals who require mental health intervention or basic needs support (housing, food, clothing, etc.).
- Communicate timely with guardians, parents, case managers, resource workers, group home staff, members, and other community agencies to facilitate mutual program planning.
- Assists in assessment and evaluation to determine member’s needs and progress relative to set goals.
- Documents observations relating to actions and behaviors and maintains clinical records of services provided to meet Medicaid billable standards.
- Continuously expands knowledge and builds relationships with other community agencies.
- Actively participates in community events to inform others of VPI and ORC services.
- Performs other duties as assigned.
PHYSICAL DEMANDS:
Functions are considered light to moderate according to the US Bureau of Labor Statistics. Work performed includes the ability to move freely (standing, stooping, walking, bending, reaching, pushing, and pulling), and lift to a maximum of twenty-five (25) pounds without assistance.
WORK ENVIRONMENT:
Work is performed in an office environment within a sedentary range setting.
JOB QUALIFICATIONS:
- Bachelor’s degree in psychology, social work, human services, or sociology, preferred.
- Minimum one (1) year experience working with individuals with mental illness or substance use disorders.
- Ability to work (1) Saturday/month (on a rotation), required.
- Ability to maintain confidentiality and operate with integrity and trust.
WAGE: 18.00-20.00/hour
